All patients were taught chest physiotherapy, bronchial hygiene, education about disease and its management and proper nutrition. All patients were assessed regarding any psychological problems. Daily 30-35 minute sessions were conducted at institute for initial 4 weeks and then home care was taught. Patient was assessed periodically for symptoms, quality of life and spirometric findings. All variables were recorded at baseline and after completion. Statistical analysis were performed using student’s‘t’ test and p values observed. Result- Dyspnea scale measured by Modified Medical Research Council (MMRC) (3.0 ± 0.66) improved after program (1.89 ± 0.83, p value less than 0.0001) which is statistically very significant. Health related quality of life measured by using St. George’s Respiratory Questionnaire 49.74 ± 5.17 improved significantly 40.92 ± 4.87 after program (p value less than 0.0001) 6 minute walk distance (280.9 ± 18.04) improved significantly after program (303 ± 17.34) (‘p’ value less than 0.0001) Spirometric parameters like FVC (p value 0.02), FEV1 (p value 0.17) and FEV1 / FVC ratio (p value 0.76) do not show any statistically significant changes after program. The dyes are both synthetic and natural, the synthetic ones may be harmful, hazardous to health and are costly to obtain. Natural dyes are eco-friendly, not costly and not hazardous to health as compared to synthetic dyes. In this study, various procedures such as H and E which serves as control as normal routine staining in histopathology laboratory was used to compare Beetroot and Eosin (B and E), Haematoxylin and Turmeric (H and T). Beetroot and Turmeric (B T) Beetroot and Eosin stains collagen fibre almost similar to H and E, that shows beetroot can be used as alternative for Haematoxylin in H and E staining technique. Haematoxylin and Turmeric stained the tissue almost similar to H and E which shows that Turmeric can replace Eosin. However, B and T poorly stained collagen fibre when compared with H and E. The conclusion came out that, Beetroot containing Betanin which gives it a red color has the same ability of staining Nuclei Bluish purple because of its basic content and may require a mordant (Vineger) as attain enhancer for most of the tissues. Turmeric exhibits the basophilic-like effect on collage fibre, it stains the cytoplasm fairly yellowish, because of its curcumin that makes it yellow. Beetroot and Turmeric as a direct replacement of H and E staining technique in Histopathology laboratory should further be studied with adequate concentration and mordant as the features is suggestive that B and T can replace H and E. Increasing age, trend of obesity and worsening renal function increasing the prevalence of hypertension in people with type-2 diabetes mellitus. We have very few research-based information regarding these issues. Aim of the study: The aim of this study was to assess the prevalence of hypertension in patients with type-2 diabetes mellitus. Methods: This was a cross-sectional study which was conducted in the Department of Medicine and Endocrinology of Bangabandhu Sheikh Mujib Medical University, Dhaka Bangladesh from January to December 2021. In total 142 patients with type-2 diabetes mellitus attended to the mentioned hospital with proper documents were finalized as the study people. Besides data regarding type 2 diabetes mellitus, data of blood pressure and macro-vascular as well as micro-vascular complications were collected. A predesigned questioner was applied to collect patient data. All data were processed, analyzed and disseminated by MS Office and SPSS version 20. Results: In this study, among total participants in analyzing the prevalence of hypertension we observed out of 142 patients with T2DM only 45 were with hypertension which was 32%. In analyzing the stages of hypertension among total participants we found, 23.39% were with normal and 35.92% pre-hypertension (In total 68.31%). On the other hand, among 32% hypertensive participants, 19.72% (n=28) were with stage I HTN whereas 11.97% (n=17) were with stage II HTN. Conclusion: In this study, the prevalence of hypertension among type 2 diabetes patients was found some higher than that in normal. But if we consider the number of pre-hypertensive patients as a potential concern, then it is alarming. The findings of this study may be helpful in the treatment arena of diabetes and in similar further studies. It's linked to the onset of secondary problems, which can lead to a variety of co-morbidities. Recent research has found that diabetics are more likely to acquire cognitive impairment or dementia. Diabetes is linked to a number of neurological illnesses, including Alzheimer's disease (AD). Evidence of a relationship between diabetes and AD is growing. Insulin signalling disruption in the brain has been discovered, resulting in increased tau protein phosphorylation (hyperphosphorylation), a hallmark and diagnostic of AD pathology, and the buildup of neurofibrillary tangles (NFT). Insulin malfunction in the brain has been shown to modify glycogen synthase kinase-3β (GSK-3β) activity, resulting in increased β amyloid and tau phosphorylation in diabetics. GSK-3β signalling has been implicated in the physiological and pathological processes of diabetes and AD, respectively. This could explain why diabetic individuals have a higher chance of developing AD as their diabetes progresses and they get older. Interestingly, several in vivo investigations with oral antidiabetic medications and insulin treatment in diabetic patients showed improved cognitive function and lower tau hyperphosphorylation. The relationship between T2DM and AD as it relates to amyloid and tau pathology will be discussed in this article. A better knowledge of the relationship between T2DM and AD could transform how researchers and doctors handle both diseases in the future, potentially leading to new therapies and prevention techniques. According to United Nations, Nepal is a poorly developed country. Due to the quality of medical care that Nepali children receive in orphanages, it has been reported that the diagnoses described in the pre-adoptive medical reports do not offer an absolute guarantee of accuracy, and sometimes these reports are not provided to adoptive families. The objective of this study was to describe epidemiological characteristics, pre-adoptive history, and medical problems observed in the post-adoption evaluation in a cohort of children adopted from Nepal. Material and Methods: Retrospective descriptive study on 11 Nepali adopted children from 2005 to 2011, examined according to a standardized protocol. Variables were collected from pre-adoption medical reports, interviews with the family, and post-adoption evaluation records. Results: Epidemiological characteristics and pre-adoptive history: females, 63.6%; all were abandoned and adopted from orphanages; mean age at institutionalization was 32 months, and at adoption was 55 months. 72.7% had a pre-adoptive medical report, and all were diagnosed as "healthy". 54.5% had a vaccination certificate. Medical problems most frequent diagnosed in post-adoption evaluation were iron deficiency anemia (72.7%), xerosis cutis (54.4%), neurodevelopmental delay (45.5%), underweight and stunting (36.4%), and acute bronchitis (36.4%). Only one child had a serious problem (cerebral palsy). The BCG vaccination scar was present in 81.8%. 72.7% showed vaccination immunity against hepatitis B virus. Conclusion: The pre-adoption medical information for the Nepali children was unreliable, and all showed more than one medical problem in the post-adoption evaluation. Adoptive families should seek pre-travel specialized medical advice regarding the medical reports and health of the child to be adopted, the updating of routine immunizations, and the destination-specific health problems. The present study was conducted to study the clinical profile and outcome and risk factors of Kawasaki disease in children. This observational prospective study was conducted in cardiology ward with a diagnosis of Kawasaki disease from 1st June 2020 to 31 July 2021 in Bangabandhu Sheikh Mujib Medical University Hospital Dhaka, Bangladesh. Forty five (45) children aged between 3 months to 10 years were studied. Kawasaki disease is more prevalent among male child (73.33%). Most commonly encountered risk factor for cardiovascular involvement especially coronary dilatation or aneurysmal change is more observed in younger age of presentation. Fever, rash, conjunctivitis, erythema and edema of limbs, periungual skin desquamations are the presenting features. Cardiovascular involvement of left main coronary artery dilation is the most frequently observed aneurismal change observed among study population which was evident in 68% of the children having coronary involvement. All the study patient received high dose aspirin (100%) and 93% received IVIG of whom 6.66% had IVIG- resistant Kawasaki disease. Commonly occurring complication is uveitis (17.77%). Among the total 45 study population 1 patient expired (2.22%).Clinicians should have a high index of suspicion of Kawasaki disease in persistently febrile patients and once clinically diagnosed, echocardiography should be done and IVIG therapy along with aspirin should be started, specially in infants (less than 6 months) as they have the higher tendency to develop coronary aneurysm.
